1115 Waiver Reentry Services
CHP partners with state and county correctional agencies to design, implement, and manage 1115 Waiver initiatives that improve continuity of care for justice-involved populations. Drawing on experience supporting programs such as California’s CalAIM Justice-Involved initiative, we help organizations build sustainable, compliant, and effective reentry healthcare programs.
